Clinical Research

Calcium plus vitamin D supplementation and the risk of fractures

New England Journal of Medicine, 2006 Randomized controlled trial (Women's Health Initiative)

A large randomized controlled trial evaluating the effect of combined calcium and vitamin D supplementation on fracture risk in postmenopausal women. Supplementation with calcium 1,000mg and vitamin D3 400IU per day produced a significant increase in hip bone mineral density compared to placebo. The study is one of the largest and most cited trials on calcium and vitamin D supplementation for bone health, supporting the Health Canada licensed claim that calcium and vitamin D help in the development and maintenance of bones and teeth

Vitamin D and intestinal calcium absorption

Molecular and Cellular Endocrinology, 2011 Peer reviewed review

A review of the mechanisms by which Vitamin D3 (Cholecalciferol) and its active metabolite calcitriol regulate intestinal calcium absorption. Calcitriol stimulates the production of calcium-binding proteins (calbindins) in the intestinal wall, increasing calcium absorption from 10–15% (without adequate Vitamin D) to 30–40% (with sufficient Vitamin D). This review provides the mechanistic basis for the Health Canada licensed claim that Vitamin D helps in the absorption and use of calcium and phosphorus

Magnesium and vitamin D deficiency as a potential cause of immune dysfunction, cytokine storm and disseminated intravascular coagulation in COVID-19 patients

Magnesium Research, 2021 Peer reviewed review

A review examining the relationship between magnesium status and Vitamin D metabolism, with particular focus on magnesium's role as a cofactor in the enzymatic hydroxylation steps that convert cholecalciferol to its active form (calcitriol). Magnesium deficiency impairs Vitamin D activation, limiting its ability to stimulate calcium absorption and support immune function. This review supports the rationale for combining magnesium with calcium and Vitamin D3 in a single formula.

Frequently Asked Questions

Why do calcium, magnesium, and Vitamin D3 need to be taken together?

Each of the three nutrients plays a distinct and interdependent role in bone mineral metabolism. Calcium is the primary structural mineral of bone and tooth tissue but without adequate Vitamin D3, the body absorbs only 10 to 15% of ingested calcium. With sufficient Vitamin D3, absorption increases to 30 to 40%. Vitamin D3 (Cholecalciferol) is converted in the liver and kidneys to calcitriol, its active form, which stimulates production of calcium-binding proteins in the intestinal wall. Magnesium is required for the enzymatic activation of Vitamin D without magnesium, Vitamin D cannot be converted to its active form. Magnesium also supports the transport of calcium into bone tissue and is itself a structural component of the bone mineral matrix. Without all three nutrients, the system is incomplete.

What is Calcium Carbonate and how is it different from Calcium Citrate?

Calcium Carbonate is the most concentrated form of supplemental calcium by elemental weight it is 40% elemental calcium, meaning 750mg of Calcium Carbonate provides 750mg of elemental calcium. It requires gastric acid for optimal dissolution and is best absorbed when taken with food. Calcium Citrate (approximately 21% elemental calcium) can be taken without food and may be better tolerated by those with low stomach acid or who take acid reducing medications, but requires a larger dose to deliver the same elemental calcium. This formula uses Calcium Carbonate take with food for best absorption.

Why should this product be taken with food?

Calcium Carbonate requires gastric acid for dissolution and ionization. Food stimulates gastric acid secretion, significantly improving the absorption of calcium carbonate. Taking this supplement with a meal also improves the absorption of the fat soluble Vitamin D3 (Cholecalciferol), which is best absorbed in the presence of dietary fat.

When is the best time to take calcium magnesium vitamin D3?

Take with food ideally with a meal containing dietary fat. Calcium Carbonate requires gastric acid for optimal absorption, which is stimulated by eating. Vitamin D3 (Cholecalciferol) is fat-soluble and is best absorbed in the presence of dietary fat. Split the dose: 1 softgel with breakfast and 1 softgel with dinner. Take a few hours before or after other medications, particularly thyroid medications, antibiotics, bisphosphonates, or iron supplements.

Why do you need magnesium with calcium and vitamin D?

Magnesium is required for two critical steps in Vitamin D metabolism: the hepatic hydroxylation of cholecalciferol to 25-hydroxyvitamin D, and the renal hydroxylation to calcitriol (the active hormonal form). Without adequate magnesium, Vitamin D cannot be fully activated limiting its ability to stimulate calcium absorption. Magnesium also supports the active transport of calcium into bone tissue and is a structural component of the bone mineral matrix, with approximately 60% of the body's magnesium stored in bone.

Who should not take calcium magnesium vitamin D3 supplements?

Consult a healthcare practitioner before use if you are pregnant or breastfeeding, have kidney disease or a history of kidney stones, have hypercalcemia (elevated blood calcium), or are taking prescription medications. Do not exceed the recommended dose without healthcare practitioner guidance. Excessive calcium intake may cause constipation and, at very high doses over extended periods, may contribute to hypercalcemia.
