Vitamin D and intestinal calcium absorption: mechanisms and clinical implications

This mechanistic review examined the role of Vitamin D3 (cholecalciferol) and its active metabolite 1,25-dihydroxyvitamin D (calcitriol) in intestinal calcium absorption. Vitamin D3 is converted in the liver to 25-hydroxyvitamin D (25(OH)D) and then in the kidney to calcitriol, which binds to the Vitamin D receptor (VDR) in intestinal enterocytes and upregulates expression of calcium transport proteins particularly TRPV6 (epithelial calcium channel) and calbindin-D9k (intracellular calcium binding protein) increasing transcellular calcium transport across the intestinal epithelium. The review documented that adequate Vitamin D status is essential for efficient calcium absorption, with deficiency associated with significantly reduced calcium uptake from the diet and compensatory secondary hyperparathyroidism.

Three year low dose menaquinone 7 supplementation helps decrease bone loss in healthy postmenopausal women

This randomized, double blind, placebo controlled trial examined the effects of 180mcg MK-7 daily supplementation on bone mineral density and bone strength indices in 244 healthy postmenopausal women over 3 years. MK-7 supplementation significantly improved vitamin K status (measured by carboxylated osteocalcin levels) and was associated with significantly less age related bone mineral density loss at the lumbar spine and femoral neck compared to placebo. Bone strength indices also showed significant improvement in the MK-7 group. The authors concluded that MK-7 supplementation at 180mcg per day for 3 years improved bone health in healthy postmenopausal women, with the effects attributed to MK-7's role in activating osteocalcin the vitamin K-dependent protein that binds calcium into bone matrix.

Combined supplementation of vitamin D3 and K2 and arterial stiffness: a systematic review

This systematic review examined clinical evidence for the combined effects of Vitamin D3 and K2 supplementation on arterial stiffness a key marker of cardiovascular health across randomized controlled trials. The proposed mechanism centers on Matrix Gla Protein (MGP), a vitamin K-dependent protein that is the most potent known inhibitor of vascular calcification. MGP requires vitamin K2 carboxylation to be active; uncarboxylated MGP (ucMGP) is unable to inhibit calcium deposition in arterial walls. Vitamin D3 increases calcium availability systemically, while K2 activates MGP to direct calcium away from vascular tissue. Several included trials demonstrated that combined D3 + K2 supplementation was associated with reductions in arterial stiffness markers compared to D3 alone or placebo, with the magnitude of effect correlating with improvements in MGP carboxylation status.
